Is arthroscopic release of the suprascapular nerve an option?
Yes-----both the safety and the feasibility of release of the transverse scapular ligament --arthroscopically--has been shown. I performed this proedure for 2 indications:
1. When there is electromyographic evidence of supraspinatus and infraspinatus fibrillation in the absence of a ganglion on MRI. In such cases, this can normalize the abnormal nerve study, and
2. In cases where the supraspinatus tendon is irreparable; in these cases, the nerve can be kinked. When repair is not an option to potentially restore a normal unkinked course for the nerve, releasing the transverse scapular ligamant has been shown to potentially decrease pain.
